Commercial drivers (CDL), or employees sent on errands, whether in company or private vehicles, the company is legally exposed. A Moving Violation Report (MVR), utilizing the driver's license number, provides a 3-7 year history of an individual's driving performance.

- Your full name as it appears on your driver's license.
